Guidance notes:

1. This form covers claims up to £20 in associated costs for one member, at one YZ event, subject to funds availability
2. This form needs to be completed by each individual member for each relevant event after successfully attending the YZ event
3. Claims must be submitted within 1 week after the event has taken place.
4. The decision to approve or refuse claim is the joint responsibility of the Chair and Treasurer and their decision is final.
5. Please remember to claim on the basis of need as funding is limited.
